Two main data sources are: the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS).
In the case of the CDC, data was collected within two main programs:
- National Vital Statistics System-Mortality (NVSS-M, CDC/NCHS)
- Behavioral Risk Factor Surveillance System (BRFSS, CDC/PHSIPO)
In the case of the CMS, data was retrieved from the Chronic Condition Data Warehouse (CCW), a data management program conducted within the CMS activities.
All of the data presented here is publically available and easily accessible via the Health Indicators Warehouse website.
